Collect & submit period of application forms：Accepting applications throughout the year

***The applicant must be at least one year old at the date of application form submission*** 

Together with the documents required:

1. Completed and photo pasted application form
2. Two self-addressed and stamped $2.2 envelopes
3. A copy of the child applicant’s birth certificate
